Information in your request
The consultation form collects your name, email, selected service, and time preferences. You can optionally provide a phone number and a short message. Please do not include medical records or sensitive health information.
Purpose and storage
Your submitted details and contact consent are stored with a request reference in the service database so the studio can review your inquiry and arrange consultation details. Booking forms never collect card information directly.
Sign-in and deposit payments
To book, you sign in with Google, which shares your name, email address, and profile photo with us. Deposits are processed by Stripe, which handles your card details; we receive only the payment status, amount, and a payment reference. Stripe’s own privacy policy applies to the payment information you give it.
Contact permission
The form asks for permission to contact you about this request. This permission is not a newsletter subscription. Submitting the form does not confirm a provider, appointment, treatment, or price.
